IRCTC Reservation and Cancellation Form 2025
Instructions were issued to Zonal Railways vide this office letter of even number dated 19.04.2025 (Corrigendum to Commercial Circular No. 11. of 2025) forwarding therewith a copy of updated Reservation/Cancellation Requisition form after incorporating the columns for exercising options for choosing ‘Vikalp’ scheme and for indicating ‘Aadhaar’ number.

- Name of applicant (or representative)
- Full address, contact number, signature, date, and time
- Office Use Section:
- Reservation clerk fills in PNR, Berth/Seat Number, and Amount Collected

- Maximum 6 passengers per form
- Only one requisition per person at a time
- Verify ticket and balance before leaving the counter
- Illegible or incomplete forms are rejected
